Summary:
The Central Intake Coordinator-Powerback to You manages the centralized referrals that are received from business partners and community sources.
The referral management process includes receiving the referral, mapping the referral to the appropriate Vitality to You location to determine if the referral is in a serviceable area, initiating the verification and/or authorization process,and then routing the referral to the appropriate resource to schedule the initial appointment.
The Central Intake Coordinator-Powerback to You will report to the Director, Customer Service Center-Powerback to You and the reporting relationship includes but is not limited to:
Timely response to referrals Tracking volume of referrals from individuals sources Conversion rates (referral to admission) Identification of admission barriers to admission and participation in improvement plans CCIV Requirements:
SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
1.
Central Intake Coordinator-Powerback to You must possess a minimum of an associate's degree in healthcare, human resources or business management.
2.
The Central Intake Coordinator-Powerback to You must also have at least 1-3 years appropriate employment experience.
3.
Experience in a rehab setting is preferred.
Compensation:
17 Timely response to referrals Tracking volume of referrals from individuals sources Conversion rates (referral to admission) Identification of admission barriers to admission and participation in improvement plans.
Estimated Salary: $20 to $28 per hour based on qualifications.
